Last Call Foundation Guidelines for Grant Applications

The mission of the Last Call Foundation is to provide funding, education and research to advance the safety needs of the firefighting community. To that end, we constantly advocate for the dedicated professionals of the fire service to effectively and safely fight fires and protect the public.


Funding Eligibility
In keeping with its mission, Last Call Foundation welcomes funding proposals to address the health, safety and welfare of firefighters. 


LCF will not provide, nor may funds be allotted to cover administrative costs for major institutions that have a sizeable endowment. With justification, at the discretion of LCF, exceptions may be made for small organizations. Conditions will be specified and agreed to upon granting the award. 


Grant applicants must attest that they do not discriminate on the basis of race, color, creed, gender, sexual orientation, disability, age, national origin or political affiliation.


Funds must be used exclusively for the purposes stated in the grant application.


Deadlines for Submission
Applications are due twice a year - April 1st and October 1st. The Last Call Foundation Board of Trustees will review the grant application, seek additional information if needed, and announce award decisions by May 31 and November 30, respectively. 


Late applications will not be accepted or reviewed. 


Exception for Critical Need  
LCF may consider an urgent critical need funding request that falls outside the application cycle. Please contact Kathy Crosby-Bell (kathycrosby@comcast.net) regarding how to proceed for an out-of-cycle request.

Proposal Requirements and Content
Applications should succinctly describe the proposed work, timeline, budget and goals—related to firefighter health, safety and well-being. 


Submit proposals as a Word document only, not exceeding four pages, following the outline below. Appended materials may be attached as pdf documents. 


General

  • Legal name of organization; address, phone, website

  • Name of executive director/director/head

  • Description of the organization (50 words or less)

  • Contact person and title; email, phone(s), fax 

  • Organization’s operating budget for current fiscal year

  • Has the organization or its board members/officers been a party to any legal proceedings in the last five years?

  • Summary of the proposal (50 words or less)

  • Funding amount requested from Last Call Foundation

  • Total project budget

  • Anticipated funding sources: LCF and others

  • Amount and dates of previous support from Last Call Foundation, if any

Proposal Narrative

  • Statement of mission for the organization; history, key achievements

  • Size and structure of the organization; list board members and their qualifications/affiliations; number of staff and volunteers expected to be engaged in this proposal.

  • Description of proposed work: 

    • Statement of problem or need being addressed; target population

    • Project goals and objectives, with milestones and reasonable projections

    • Expected deliverables and dates

    • Anticipated outcomes or benefits

  • Budget allocation: How funds will be used to complete the project; include line-item expenses

  • Methodology: Brief outline of the phases and associated budgets, with timelines; allocation of staff

  • Evaluation: How will success be measured?


Required Attachments

  • IRS letter confirming tax-exempt status, i.e., 501(c)(3) or 509(a) if applicable

  • Most recent independent audit or account review and/or year-to-date financial statement for the current fiscal year

  • Indicate other organizations, foundations and sources being approached to fund this project (if applicable), along with dollar amounts and status (committed, pending, or anticipated). Include contact information. 

  • Letters of recommendation in support of proposed funding from appropriate fire service, medical, mental health, safety officials or other experts

Disclosure
Submission of a proposal does not guarantee funding. Last Call Foundation reserves the right to fund or reject a proposal, without specifying a cause.


Last Call Foundation Expectations Upon Awarding of Grant
Funds must be used exclusively for the purposes outlined in the grant application. Applicant and the funded institution will be responsible for ensuring strict adherence to the award agreement.


Once grants are awarded, detailed deliverables/reports to be presented to LCF at least quarterly. An extension may be requested for cause. However, applicants understand and agree that any extension of more than three months will require LCF approval in order to continue funding. A letter of explanation for the extension request must be submitted to Kathycrosby@comcast.net and copied to billpay@lastcallfoundation.org for consideration.  


Research Grants Results, findings, conclusions and recommendations at the completion of research projects to be submitted to LCF at least 45 days in advance of any publication. 


Equipment Grants Proof of equipment purchase and installation to be submitted to LCF prior to final payment, but no later than 30 days after delivery. Members of Last Call Foundation will have the option to be present for dedication of equipment, and placement of a plaque acknowledging the donation.


Service Project Grants Results of the funded program to be submitted to LCF within 30 days of completion.


Use of Last Call Foundation’s name and logo in promotional materials must be approved.
